We are excited that you are interested in enrolling your child(ren) in our Early Head Start/’Head Start Program. We want to assist in making this experience a lasting one.
NSPHS take many factors into consideration in order to determine eligibility. In addition to your child’s age and income level, other family needs and criteria are considered. Slots are filled based on greatest need.

The following information will be used to help determine eligibility.
For your child to be considered for enrollment at NSPHS, you will need to provide one of the following documents for enrollment.
Income verification:
- A copy of 1040
- Pay stubs (past 12 months)
- W-2
- Foster Care Reimbursement
- TANF (Department of Human Services-F.I.A.
Birth Verification:
- Letter of Birth from Hospital
- For Headstart - Child must be 3 years of age by 1st day of attendance
All head start (ages 3-5) children MUST have a current and up-to-date immunizations and yearly physical exam along with the following tests:(TB, Lead, Hemoglobin/HCT, Sickle Cell, Hearing and Vision). It is also required that you have a dental examination twice a year for your child.
*All early head start (ages 6 weeks-30 months) must have a current well baby exam and immunization records.
